• Task Request Form (Task Tracker) Task request form to request changes to SMMS or MRS.  Part of the Microsoft Planner task-tracker system.
• Supplies Order Form (Rack Cards, etc.) Excel spreadsheet to be used by SMCs to order rack cards and other materials.
• Name Badge Order Excel spreadsheet used by operation managers to order missionary name badges.
• Certificate of Service for Senior Service Missionary Sample certificate of service emailed to each senior service missionary upon release.
• SSM MLS Position Request Form Fill-in form used by mission leaders and stake presidents to request approval of a couple to serve as member-and-leader-support (MLS) senior service missionaries.  Submit to support advisers to request approval of Missionary Department and Service Mission Office.
